- Transcript of Records (TOR) / NCAE Result
- Valid ID
2. 6 pcs. passport size picture (White background, with Collar and Name Tag)
3. 6 pcs 1x1 Picture (White background, with Collar and Name Tag)
4. Long Folder
5. 1/8 index card
