• NSO Birth Certificate
• High School or College Diploma or Transcript of Records
• Passport size picture
• Police Clearance
• Long Brown Envelope
• NBI Clearance
• 1E1/SSS ID or Form
• Philhealth
• Must be a registered voter of Rodriguez (Montalban), Rizal
• Xray (Result (for Massage Therapy)

From Bayan or San Isidro, sakay po kayo ng Jeep or tricycle papuntang Bukid/Kasiglahan. Baba po kayo ng Catholic Church dito sa Kasiglahan. Then sakay po kayo ng Padyak or pedicab. Sabihin nyo lang po sa Driver na sa Tesda po kayo.
