Orientation will be on March 6, 2020, Friday, 9:00 AM.

Requirements: 

1. Must be 18 years old and above.
2. Photocopy of the following:
- NSO/PSA Birth certificate
- Marriage Certificate (if Married)
- High School/ College Diploma
- Transcript of Records (TOR)
- Form 137/Form 138 (if High School Graduate)
- Valid ID
3. 4 pcs Passport Size Picture
(White Background, with collar and Name Tag)
4. 6 pcs 1x1 Picture
(White Background, with collar)
5. Long Folder
6. 1/8 Index Card
