1. Photocopy of Birth Certificate
2. Photocopy of Any Valid ID
3. Barangay Clearance (For TESDA Purpose)
4. Photocopy of Vaccination Card (Fully Vaccinated)
5. 1 pc. 2x2 Picture white background
6. 3 pcs. 1x1 Picture white background

Note: **Bring your own Ballpen***
