QUALIFICATION:
Any of the Following:- Must be High School Graduate
- At least 15 yrs. of age who are not employed, not in education and not in training (NEET)
- Displaced workers/affected individuals due to calamities/disasters
- Long-term unemployed
- Employed workers who intend to develop and expand their current skills and training, except existing workers in enterprise-based companies or industries currently trained by their employees
REQUIREMENTS:
- Photocopy only (Please also present original copy upon submission of requirements)
- Form 137 or 138 for High School Graduate - 3 copies
- Transcript of Records (TOR) for College level or Graduate- 3 copies
- Birth Certificate/ NSO/ PSA - 3 copies
- Marriage Certificate ( if Married) - 3 copies
- Any GOVERNMENT Valid I.D - 7 copies
- 1 pc long brown
- 4 pcs. long plastic envelope
