Training Requirements:

1. 2 pcs 1x1 picture 
2. 3 pcs passport size id picture with a name tag and in business attire 
3. 2 copies of 1 valid ID with 3 specimen signatures 
4. 2 copies of Form 137 or 138 if High School Graduate 
5. OR Transcript of Records if college level/graduate 
6. 1 copy of Birth Certificate 
7. 1 copy Marriage Certificate for Married Females only
