- Photocopy of Marriage Certificate (for married women)
- Photocopy of Diploma or Transcript of Records
- 2 pcs. 1x1 ID photo and 4 pcs. Passport-size photo (collared with white background and name tag, including last name, first name, middle initial, and signature)
