- A Student Permit
- A completed TESDA Application Form (MIS 03-01)
- Certificate of Completion for "Practicing COVID-19 Preventive Measures in the Workplace" (available on the TESDA Online Program website: www.e-tesda.gov.ph)
- A Medical Certificate confirming you're physically fit for Driving NC II training
- Four (4) 1x1 photos (with name tag, collar, and white background)
- Four (4) passport-sized photos (same requirements as above)
- A photocopy of your Birth Certificate
- A photocopy of a valid government ID (with 3 specimen signatures)
- Transcript of Records for college graduates or Form 137 for high school graduates
- A Diploma (whether from High School, College, or ALS)
