- Birth Certificate (original & photocopy)
- Marriage Certificate for married women (original & photocopy)
- School credentials (photocopy) such as Form 137/138, Transcript of Records (TOR), or Diploma
- Three (3) 1x1 ID pictures (white background, with collar)
- Three (3) passport-sized ID pictures
- Submit your documents in a long brown envelope
