You must submit the following documents:- Copy of PSA Birth Certificate
- Marriage Certificate (for women only)
- Medical Certificate (certifying you’re fit to work or train)
- Proof of education: Form 137/138 for high school graduates, or Transcript of Records (TOR) for college graduates (2 copies)
- Two (2) passport-sized ID pictures (white background, collared shirt, with a name tag)
- A long brown envelope to compile your documents

This scholarship is for Parañaque residents only, and proof of residency, such as a residential ID or address certification document, is required.
